MDM Trouble Report for 2021 Dec 02

Problem(s) Encountered:
Encountered a few issues with the guider/probe this evening. 
The guide probe stopped moving twice (according the the MIS GUI). 
This was fixed when we restarted the MIS GUI and power cycled the 
MIS controller. Separately, we lost guiding approximately a dozen times 
over the past two nights in clear conditions. These incidents were clustered, 
and the star jumps well out of the box (although we are aren't sure that it is a 
consistent direction).
